You may also call 1-800-871-0711 and speak with a representative from 7am to 11pm Sunday through Thursday or 7am to 1am on Fridays and Saturdays. After hours please call 1-800-871-0711, dial extension 5200 and leave a voicemail with your name, Player Card account number and address, and we will mail you the form.
